Thanks for all the replies. We called a friend who my wife went to high school with and who is now a gastrointestinal pediatrician. She said to keep up with the reflux medicine we’d been prescribed and give it a chance. She also suggested getting him tested for celiac disease since it’s an easy blood test (gluten sensitivity I think). She wants us to make a log of when he throws up and how long since he ate and what he ate etc and call back in a few weeks. She was not too worried and said that since he’s growing, eating, playing and generally well, it’s very likely not too serious. Immediately after he gets sick, he’s back to his normal self like it was a relief for him to get it out.
Some of what y’all have posted sounds like us. When he was younger he had a weird gag reflux and sometimes threw up when eating. Now he eats really fast and throws up big pieces so maybe he’s overeating and not chewing enough sometimes. His nose is also running a lot now and he wouldn’t let me put him down tonight like I always do. He cried for mom. So maybe he just has a bug right now. Its just frustrating because he will be fine for a couple months then get sick a handful of times in a twoish week span. It’s hard to put a finger on what’s causing it but it’s just often enough to make us worry.

That's frustrating for sure. We have been there, done that. Our daughter would have periodic bouts of severe vomiting and severe abdominal pain from around 5 years old through around 9 years old. We were at the pediatrician office too many times to count and the ER several times because we would go to the pediatrician and be sent to the ER to r/o appendicitis. We finally got the answer when she was around 6 years old and everything else had been ruled out... cyclic vomiting syndrome. Your kiddos problem doesn't sound like this though. It is pretty severe and she definitely would not go back to playing and it would last for days sometimes...
But I understand your frustration. Keep that log as a start for sure... hope it doesnt take yall as long to get an answer!
