Knowing that it would probably be a good five to seven min before you saw your waiter again, you would still ask to break the five?

I would ask another waiter, or the manager for change. A good manager will ask about your experince since they are having to break the $5. I've explained to restaurant managers my experience before, both good and bad, especially at places I'm I regular at.

Before anything starts, there is a 15% tip line. The waiter has full control in determining if that value increases or decreases. I have left a tip of $0 maybe 3x in my life of 47 years.

I've learned our own attitude comes into play as well. If that happened to me today, I would have asked for change and left a couple bucks. On a Monday, who knows?

True, but it sucks when you can only get one bowl and they won't bring another.

Service didn't seem too good either. Was it busy?

Maybe it's just me, but to leave no tip means the service absolutely sucked. You did get your food on time (even if not from that waiter) and an eventual drink refill.
That feels like rewarding mediocrity to me. No tip

I believe the starting point is 0% and you earn everything you get. No free rides. Earn it.

All that being said, generally I tip 20%, but again. Must meet expectations to get that. I would not have left a tip in this case

Tips are for great service. If you do not get that type service, then a tip is not in the offering.

I'd have asked for someone to break the bill so I could leave a couple bucks; OR I would have just left the $5 if I was in a hurry. Only thing worse than bad service is a cheapskate. I would never have left without leaving some sort of tip - especially if the good was "good." Don't forget, waiters have to 'tip out' other staff, depending on the restaurant setup. Some of the money may be going to the guy that cooked the good food or a busser who brought a refill. Who knows what was going on with the waiter today. It would take exceptionally bad service AND food for me not to leave a tip. I might individually tip a busser or talk to a manager and make sure my tip went to everyone BUT the guy providing bad service; but simply leaving without leaving a tip is 'bad form.' JMO
