An arrow passing thru very low can produce exactly this result. A hole in the rib cage below lungs leaves a pretty clean arrow but the diaphram sucks and blows air through the hole generating bubbles. This eventually results in the partial collapse of at least one lung. It may or may not be fatal, but most of the time, a deer will just stop and stand in one place trying to breathe.
