    It looks like the cylinder timing was off by about half a round. Crazy stuff.

        As far as post #8- yeah the cylinder and topstrap blew off because of the ammo load. We have one here in our lab that looks very much like it. If you use more than the recommended powder loads this is the risk you take. S&W is a solid piece of hammer forged steel. Notice how the cartridge next to it blew off, but the bullet remains? That is what happens when the cartridge is not supported by the chamber.
        I used to own a 686 and they are tough!!!!
