Not my gun but the guy shooting it got banged on the forehead from the top of the cylinder but it didn't knock him down or hurt him badly.

hmm.. how is that the ammo's fault? looks like the cylinder breached..and blew apart..

Cheap ammo or cheap ammo reloading manual?

Yeah looks to me like someone more than doubled a reload to cause the cylinder to breach.

I have seen this pic or one just like it and a guy put the maximum reccomended load in his pistol (can't remember the manual) and this is what happened. The next edition of the manual had a lower max charge.

Are you kidding me? How many times is the story on this gun going to change? It was posted here a few months ago with another story attached to it. I saw the pics well over a year ago with a different story of having been brought to a police station. The box it sits in- it is a gun evidence box. That is the only true thing about the stories I have heard.

As far as post #8- yeah the cylinder and topstrap blew off because of the ammo load. We have one here in our lab that looks very much like it. If you use more than the recommended powder loads this is the risk you take. S&W is a solid piece of hammer forged steel. Notice how the cartridge next to it blew off, but the bullet remains? That is what happens when the cartridge is not supported by the chamber.
