Supressor Questions

Is this the form I need to fill out if building a suppressor:

I know I have to apply and be approved before building. Just want to make sure it's the right form.

You should be able to do it online. Should be form 1.

No that’s a form 4. You need the form 1. Bottom right hand corner.

OK, thanks. I think I figured out how to apply online.

So... another dumb question. On the form 5320.23, I assume I am the responsible person? Probably shouldn't be that confusing but it just seems redundant.
