Form 1 Help!!!

So I sent my form 1 in last year before 41F hit. I received the form back this week to fix a couple of things. I called the ATF because I had a question about one of the updates that needed to be made. She was no help at all and now im slightly confused.

My question is for the Maker's Certification section # 11.- 13 if I am filing as a trust do I need to answer those questions?

Also, on the bottom of page 2 do on the signature of maker line do I sign my name or leave it blank?
