I load from the slide locked but that's just between mag changes.
When you release the slide, do you ease it with your had or just let it slam forward?
I'm curious if it will discharge that way?
In 1990 while in the Marines we shot the Beretta 92FS 9mm. We'd insert the magazine, rack the slide to chamber a round, during reloads the slide would lock to the rear,, we'd insert new magazine, release the slide and resume shooting.
I'm not overly familiar with the 1911 platform
I prefer to rack the slide by hand (regardless if already locked back or empty & closed) & let the full throw make a solid chamber...the 1911 has a good safety design & unless you are careless, a discharge is very unlikely.

Ditto
Any semi auto I rack the slide by hand each and every time. First time/slide down/slide locked back/clearing jams/misfire/don't matter.
And carry cocked and locked. Practice un-holstering, taking safety off and no finger on trigger (empty chamber no mag). And practice, practice, practice. And then some more. Make it second nature so that even in a stressful situation your training and muscle memory won't fail you. Maybe have your wife occasionally scream and holler every so often. If the hammer drops, practice more. AND WITH AN EMPTY CHAMBER AND MAG!!

Always let it slam forward unless on an empty chamber.
Don't ease it forward. You risk it not going completely in to battery that way. Also, don't drop a round into the chamber and let the slide forward on it, you can break the extractor like that. Many prefer do release the slide from slide lock by pulling the slide back a hair after inserting the full mag, rather than using the slide release. Matter of preference there.
