Help Needed to Identify Root Cause of a Issue We had

My son was given a new to us PSE bow, it appeared to be in great shape and well taken care of. He has shot the bow atleast a half dozen times and probably ran 50ish arrows through it. Then the last time he mentioned some noise. So we went out to shoot together and over the course of a couple shots, I did hear the noise. Once we inspected an arrow and the end was split near the nock, which I find odd. Then he proceeded to take another shot and the bow "blew up" and the strings came off, etc. Lucky he was not hurt. I inspected everything and plan to take it into a bow shop to further inspect, re-install strings/cables and also ensure that its properly tuned. However, what I am at a loss for is "why" did this happen. Anyone ever have anything like this?

Here are the facts that I know of:
    1. There was a noise on a couple shots during the draw cycle
    2. He had 2 arrows with cracks in the end , near the nock
    3. The bow blew up in his hands, either at full draw or near the end of draw cycle.
    4. the bottom end of the d-loop had come off

    So my thoughts.
    1. could the d-loop have come loose and caused the bow to blow up?
    2. split of the arrow. Could this be due to some amount of torque by the release?
    3. also could this have cause the string to jump off the cams? Is it possible to induce torque.

Here are my decently educated opinions.

    1. could the d-loop have come loose and caused the bow to blow up?
My opinion- no. I'd look more toward hand torque, just a guess without seeing his draw cycle, or cycle on a draw board.

    2. split of the arrow. Could this be due to some amount of torque by the release?
You mentioned a "used bow", were the arrows also used, and could they have been already split at the nock- i.e. basically a partial dry-fire?

    3. also could this have cause the string to jump off the cams? Is it possible to induce torque.
Again, possible "dry fire" if the nock end was already split? Hand torque?

Not sure what PSE model it is, but if cam modules- are the modules tightened up and in the proper slots, that is usually a nice source of noise.

Were the arrows underspined? Still not sure that would cause them to be split on the nock end.. maybe they were old. I’ve had many d loops break and never caused the bow to de-rail or blow up. I’d re string it and make sure all modules on the cams were tightened properly and get some new arrows. I’d make sure my nock fit on center serving is good as well
