Bill Of Sale Question - What Has To Be There?

I sold my kayak trailer (homemade) to a guy and need to write a bill of sale for him to get it registered.

What has to be included in a bill of sale for something like this? It's just a small 5x8 flat trailer with a wooden kayak rack built on top.
