Waking up from an induced coma

My brother had a heart attach. They worked on him for about 90 minutes. He coded 3 times and they were going to call him 2 times. Each time they heard a faith heart beat. He has been in ICS in an induced coma for since Monday. They reduced the sedation meds yesterday and stopped them late last night or this morning. So far he is not showing any signed of waking up. What is the process if he does not wake up.

He is not in great heath. Previous heart issues. low level diabetes, bad circulation on both legs, Already lost 1/2 of left foot. and his legs current could be causing some other issues. They say he had a blood infection that might have caused the heart attach. They did culture his blood and said the bacteria was not identified.
    DR to overlook
Since he was worked on for so long they are concerned about brain damage but have delayed doing any brain activity test. His son and I do not want to make any decisions on care without knowing about brain damage.

So much unknown and he has lots of issues.

Just need some input from people in the medical field.

He had many doctors right now. Critical care, heart DR, kidney DR, infection doc. surgeon, Etc
