I don't remember the form, but if you order a Mckenzie catalogue you can easily find it. my wall mount is also a pedestal mount and Biggs (on TBH) who did the mount made a custom cedar pedestal that is shaped like an arrowhead, but I do not have the space to use it right now, so it's on the wall.
