How does a navy ship collide with another ship?

When my brother was in the Marines, he was off of the coast of Hawaii when the ship he was on hit another one. I can't remember if it was a refueling thing or what exactly happened.

Im assuming that there is highly sophisticated radar on board and that if being used it is impossible to hit another ship.
But, i have NO idea what tech is on a ship
