Guy is clearly psychiatrically ill, and was still able to buy a gun -- why didn't the background check catch this? I'd bet because the mental health professionals didn't want to risk a lawsuit by reporting him or otherwise insuring his mind-state was in the database.

We don't have a perfect system...NICS failed on the recent racist church shootings. The suspect got a delay on his 4473 & had an arrest on record (not enough to keep him from being denied) but NICS should have looked deeper & seen that he 'admitted' to drug possession which would have been sufficient. The time table passed & the FFL was legal to allow him to take the weapon.
I suspect NICS had nothing see in this case as you have mentioned...they use three different computer systems to do the background checks & for the most part are very reliable.
I'm betting mental illness will continue to be an angle the anti's will use...problem is unless it's reported to proper authorities, we will have to relinquish our liberties to get perceived safety for anymore new laws.
Bottom line is you cannot legislate or control deviant behavior without losing all aspects of freedom & banishing the 2nd.
