Only time my grates don't have rust on them is when I bought my grill and pit. I didn't even know they weren't supposed to rust! If I threw out my rusted grates, I would have to win the lottery to just keep up with replacing them!!!!! A little heat and a quick brushing and they are good to go! Heat kills the bacteria and loosens the crud, brush knocks off all the loose stuff so it doesn't stick to the food. Other than that, most people need more iron in their diet anyway!!! Rust is harmless. Its the bacteria it traps that is bad and if you aren't getting it hot enough to kill that, you aren't cooking your meat enough!!!
