Need help from a Database SME.

I have this friend who is a school teacher and she has started to build a Database for her school and was wanting to add something to her database but not sure how to go about it. I told her I may know of some people that could answer her question. So I am here hoping someone could help this teacher out.
    Here is her question:

I'm adding some new things to my program for next school year, but I may have a vision larger than what I am capable of since I first heard of databases this past fall and kind of figured it out on my own.

I’m thinking of uploading a document and making it a form by itself in the database where the counselor can just open it up and see the grades and it look like what she's been working with over the years. We teachers will be inputting the final grade in a different form, though.

Where I get stuck is trying to think of how to break it down in my tables, and I’ll do my best to explain so it makes sense. All of this will be used to determine which classes the students have taken already and which they have left remaining as well as see their final grades – so basically this whole document will be filled by the time they graduate.

I have a table for departments and a table for courses. I was wanting to make one table to collect all the final grades as the teachers enter them, but I’m pretty sure that won’t work. So now, I’m thinking I should make a separate table for each department with the keys being department and student (so English is one table, math another, science another, etc). Then make a query that brings them all together to go into a report or form. Is there an easier way (assuming you followed that)?

If that’s how to do it, then would I use that same query to make the form the teachers use to enter the final grade into?

I always have a vision of what I want the end product to do and look like, but sometimes I struggle with breaking it down to the smallest detail, and this one is doing so many things at once that I’m unsure what to do.

    Any advice would be great!

We figured it out today. Took a while with different steps but we were able to get it to do what she wanted.
