60 rounds aint enuff to make it that dirty. I've ran 100's of round through mine in the field when I was in the Corps and for the most part, didn't have an issue. Granted, these were Colt M-16A2's.

If so, I'm going with loose gas block. It could be overgassed and running too fast, not allowing the next round to come up before the bolt goes forward, but that would have been from round one, not 60 rounds in. It could be under gassed, thrusting the bolt back enough to eject, but not far enough to catch the next shell. Best way to tell is where the spent cases land. I'm going with under gassed due to a loosened and leaking gas block since it worked for a while, then stopped working, and since you have unreliable extraction, overgassed would eject well. If it were a buffer problem, it would have been a problem from the first shot.

I cant tell you where the spent cases were landing I never looked at that. Sometimes it would jam up also not ejecting the spent round and hang half open.

Thanks for all the input. I could not take it anymore. Stripped everything down and 're did everything. Only thing I found loose was the Bolt Carrier Key so I tightened it back up and put enough oil on her to make a rusty wrench happy. Back at the range now and everything seems ok with 40 rounds down range. Thanks again for all the help.

A loose gas key will cause failures every time.
With that said, the bgc I received with my Sabre defense upper from Red-x arms is weakly staked to say the least. The marks are there, but my other BCGs are staked much deeper.
How do I stake it deeper? Is it as simple as pulling out the ole punch and ball peen and giving it a few whacks each?

it may be ok, it doesnt take much to hit the spec.
