A couple of my associates use the Dragon Dictation app on their iPhones and say they really like it and that it is pretty accurate.

I bought it for my wife when she was writing long essays and what not. She didn't care for it much, it was very laggy, and constantly used the wrong words/context.

Yes I used to use it all the time at my last job... Not terrible if you take the time to train it... I thought the iPhone app was a little better than the computer version to be used with a headset though
