Mechanics help me diagnose a running lean error code on explorer

Had a problem on the explorer a few months ago where I was getting error codes for multiple misfires and a lot of oxygen sensors codes. Since then I have changed plugs and replaced the oxygen sensor. Now I still get an error code showing the explorer is running lean. What should I try next? I was thinking of cleaning the MAF and see what happens but honestly i don't think that's the problem. Idles fine, etc. Should I spray carb cleaner on the ends of vacuum hoses to see if it revs up for a vacuum leak? Should I replace the PCV value?

The explorer is 8 yrs old and has 125k miles on it. It's giving error code P2195

Try the vaccum leak test then change fuel filter or clean injectors

it also stalls out/loses power every once in a while while driving. Does this sometimes when its under a load (going up hill, giving gas, etc.) where engine completely shuts off. Put in N, restart, drives fine from there.

It doesn't do it often and I've driven it 100 miles the last two days and its only done that to me 2x.

Sounds like a vacuum leak to me, but there are other things it could be.

              Do you smoke?

                P2195 - O2 Sensor Signal Biased/Stuck Lean - Bank 1, Sensor 1
Check for Exhaust leaks before the o2 sensors, vacuum leaks, PCV system or EGR system leaks

I'm leaning toward vacuum or fuel system with vacuum at the top. I think the PCV valve is less than $50 so I may be replacing it this weekend and go from there.

                    smoke what?

j/k. I don't smoke. I'll get some carb cleaner and spray on the ends and see if it bogs down/revs up

No exhaust leaks that I know of or could see when I crawled under it. Replaced the o2 sensor that was showing multiple codes over 300 miles ago. Next was thinking of replacing the PCV system. Guess I'll stick to my plan of action. I can do the work, just not great at diagnosing the problem!

I work in service for a Ford dealer and will check with one of my tune-up techs tomorrow and get back with you.
