anyone ever have a dealership tell them their vehicle repair was not covered because they said you failed to maintain the vehicle pursuant to manufacturer specs?

The dealerships do have that option, usually, at least with Chevrolet when I was working there, they would have to identify a lack of maintenance being the specific cause of failure.
We had one guy who didn't change the oil on his Aveo for 22k miles and seized the engine. It was the direct cause of failure.

If the vehicle is out of factory warranty but still under extended warranty, take it to a different dealership than where you bought it. I bought an extended warranty through the dealership I bought my last truck from and was given the run around. Called another dealer, they looked up my warranty and said it was covered. They fixed in two days and got loyal customer in return.

Don't know what manutacturer you're dealing with, but the dealer does not have the last call.
Example: I have an old Jeep Liberty (long out of warranty), diesel, up armored with full skid plates, lift kit, etc. that I dearly love. Keeps having problems with the side glass power windows losing their brains and falling down. Being as how there has never been a recall on the problem, the dealer charges (last time, this is number five, and fairly representative) $386.10. I call customer Support at Chrysler/Jeep, talk to the rep, send them a scanned in copy of the bill via the net, and they send me a check. Just waiting on my latest check. Should be here any day.
Does it apply to your situation? I have no Idear, but worth looking into.
