We had it done on our old house about 20 years ago. Ask about a warranty first thing. Also, time the job will take & ask about clean up. I think most companies will be good on those things. I can't remember anything else. We were very pleased with the bunch that did ours.
I bought a house not long ago, and had foundation work done. In my opinion about the most important thing is that you get a Life Time transferable warranty. If the house starts to settle again it will be under warranty and they would have to fix that, and that will go a long ways if you ever decide to sell your house. Check that the company has been in business, and registered in the state. I ran into some that came by to give me estimates that were registered or had insurance. A Life Time Warranty from a company that might not be in business further down the road....well you get my drift.
Ask if any plumbing line breaks-(Supply or drain line) caused by the repair or foundation lifting will be repaired or if it will cost extra.
I forgot to mention this one......yes ask about the plumbing, and your roof if your house has to be brought up alot to level it. When i asked that, i made them write it in their estimate, that they would be responsible.

I've got the name of one place here that I've heard does great work.... but are involved in a lawsuit because they won't come back out on a repair of a job that bought the Lifetime Warranty...
One thing to remember is "Was it level when it was poured"??? If the doors and windows open and close and just a few cracks in the rock it may not be to bad or need many piers.
I hired a PE to do the report and sign off the work that the contractor I hired did. This was on a house i was getting ready to sell. I had bids from some of the well known companies and from a independent and ranged from $7500 to $12000! I got it done for $2300 and that included re-grouting some of the brick work.
Ask about what happens if a water or drain pipe breaks when they start jacking thinks up. Ask about glass breakage. Well, you can ask, but it will be on your dollar to fix.
Just remember, it does not have to be perfectly level.
