PDF File help

I've got a PDF file that I would like to be able to type right into the blanks. I have an older version of Acrobat Professional but I'm having problems with it. If someone can make it where I can fill out the blanks on the form, I would appreciate it. Let me know and I will email it to you. Thanks.

I think I figured it out with the Acrobat Pro software. First time I've really used it. I'll see if it will print and take yall up on the offers if it doesn't work. Thanks alot.
