Question for CHL holders

In the list of things to mail in with your form one of them is a photo holder, what exactly is that??

It should come in your kit you get from the chl instructor.

I am an instructor and dharrisis is correct your instructor should have given you this form but things happen. Here is the link from DPS.

It is usualy a post card size and type that has 4 slots at angles that you put the 4 corners of the photo in. is may just look like a blank card
